Which insurance plans does Paradigm Psychiatry accept?

Paradigm Psychiatry is contracted with the following insurance plans:

  • Cigna
  • Aetna/Banner Aetna
  • Blue Cross Blue Shield (BCBS)/AmeriBen
    • If your BCBS plan is out of state, please contact your insurance to verify benefits
  • United Health Care/Optum Behavioral Health/ UMR
  • Tricare West

What insurance plans are you NOT contracted with?

We do not accept the following insurance plans and patients will be required to pay the SELF PAY rate to be seen.
(Intake-$250, Follow Up-$150)

Paradigm Psychiatry is NOT contracted with any MEDICARE/MEDICAID plans.

Paradigm Psychiatry is NOT contracted with any of the following AHCCCS plans:

Care1st Health Plan, Health Choice Arizona, Molina Healthcare, Mercy Care, Banner University Family Care, UHC Community Plan, Arizona Complete Health.

Paradigm Psychiatry is NOT contracted with AmBetter, Gilsbar or ComPsych.

Paradigm Psychiatry is NOT contracted with BCBS (FEP) Federal Employee Program or BCBS PCP HMO Plan.

*Paradigm Psychiatry does not participate in any Workmans Compensation cases.


What are the fees for self-pay patients?

For self-pay patients, the fee for a new patient visit is $250, and the fee for a follow-up visit is $150.


Can I request medical documents/forms such as disability or a leave of absence?

Medical Forms, such as FMLA or Short Term Disability will only be considered for patients who have been established under the provider’s care for at least 90 days.

An appointment is required to with your provider to evaluate your current status, capabilities and limitations.

A $50.00 Administration Fee is charged for completion of all forms, documents, or letters.

Paradigm Psychiatry does not complete Long Term Disability requests.


What is the recommended arrival time for new patients?

New patients are encouraged to arrive 15 minutes prior to their scheduled appointment time to allow enough time for the registration and check-in process.


What happens if I miss or late-cancel an appointment?

First late cancellation/missed appointment: Courtesy reschedule.

Second late cancellation/missed appointment: $50.00 fee will be applied to your account.

Third late cancellation/missed appointment: $50.00 fee will be applied to your account and may result in discharge from the practice.


What if I am running late to my appointment?

If you are more than 10 minutes late to your appointment, please contact the office by phone at (480) 613-9599 to notify them as soon as possible. Paradigm Psychiatry has a late arrival policy, and you may be asked to reschedule to minimize wait times and provide quality care to all patients.


How do I request a refill of my medication?
  • For effective treatment, medication refills are typically sent during appointments. Your provider will send enough medication with refills to last until your next scheduled follow up. Please ensure your follow up visits are scheduled timely so you do not run out of medication early.
  • You can request a medication refill by contacting the office by phone [(480) 613-9599], email info@paradigmpsychiatry.com or send a request via patient portal.
  • For Controlled Substances, such as stimulants, an appointment is required every 1-3 months for refills.

Paradigm Psychiatry does not replace any lost, stolen, or damaged Controlled Substance Medications.
