Missed Appointment (No Call/No Show) and Same Day Cancellation (Less than 24 Hour notice)
Patients must provide a minimum of 24-hour notice to cancel an appointment, otherwise it will be recorded as a Missed Appointment/Same Day Cancellation.
To provide notice of a cancellation: Patients may call the office main phone number (480) 613-9599, if you do not reach a representative, please leave a detailed voicemail including your full name, date of birth, date/time of the scheduled appointment and reason for cancellation. Please leave a call back number so we can reach you to reschedule; voicemails can be left during or after hours. You may also send a message via online patient portal account or send an email to info@paradigmpsychiatry.com.
Please Note: Do not respond to the system automated text message Appointment Reminders. The office does not receive these messages.
Late Policy: If you are running late to your appointment, please contact the office by phone to notify us as soon as possible. In our efforts to provide quality care to you and other patients being seen, as well as to minimize your wait time, our office has implemented a late arrival policy. If you will be more than 10 minutes late to the appointment, please be advised you may be asked to reschedule.
Missed Appointment Policy:
Missed Intake- $75.00 fee will be applied to patient account and must be paid to reschedule.
1st late Cancellation/Missed appointment- Courtesy reschedule - No fee.
2nd late Cancellation/Missed appointment- $50.00 fee will be applied to patient account, card on file automatically charged.
3rd late Cancellation/Missed appointment- $50.00 fee will be applied to patient account, card on file automatically charged and may result in a discharge from the practice.
