Before your Appointment
- Please email a copy of your insurance card (Front and Back) and a Photo ID/Driver's License (Front and Back) to info@paradigmpsychiatry.com (If you are not the subscriber on the insurance card, please include their Full Name, Date of Birth, and relationship to the patient)
- You will receive an email invitation to register your online patient portal account, please log on and complete all assigned Documents and Consent Forms under 'Questionnaires'. All documents are required to be completed electronically and submitted a minimum of 48 hours prior to scheduled appointment time.
- Please arrive 15 minutes early for the check- in process. Bring a copy of your insurance card and photo ID.
- Please bring a copy of any medical records with you if necessary for the appointment, such as, previous psychiatric medical records, recent lab results, hospital discharge summary, etc.
