Disability

• Paradigm Psychiatry requires a patient to be established with the practice for a minimum of 90 days before any disability requests can be completed.
• A $50.00 Administration Fee is charged for completion of all forms, documents, or letters including FMLA, Short Term Disability or ADA (Americans with Disabilities Act)
• If requesting a leave of absence or disability, an appointment is required with your provider to evaluate capabilities and limitations, in addition to regular scheduled follow up appointments determined by your provider.
• Paradigm Psychiatry does NOT complete Permanent or Long Term Disability Requests.
