Financial

• All patients are required to have a saved credit, debit, or HSA card on file. Payment for the amount due is automatically collected at check-in prior to seeing the provider.
• An invoice is sent out every 5th of the month, your remittance is due within 10 business days from when the invoice was sent out.
• If your account becomes past due for more than 30 days, we will attempt to collect using the saved card information on file per Paradigm Psychiatry Financial Policy.
• Paradigm Psychiatry is not responsible for any overdraft fees.
• If payment is still not made due to insufficient funds or invalid card info; we reserve the right to pause all future services until payment or partial payment is made, as described in our contract terms.
• By these actions, we will only provide emergency care should the patient require it within thirty days from the date of this letter.
