- All minors (17 and under) must be accompanied to all appointments (both In-Person and Tele-Medicine) by a parent or legal guardian.
- If parents are divorced/separated and there is Joint or Sole Custody of the minor, A copy of the Legal Court Custody Agreement is required to verify who has medical decision-making rights. Please email a copy of the Legal Court Custody Agreement to info@paradigmpsychiatry.com.
- If parents are divorced/separated and share Joint Custody, consent for treatment is required from both parents.
