- New Patients are required to register an online Patient Portal Account through the electronic health record. After the Intake appointment has been scheduled, patients will receive an email invitation to register their account.
- All New Patient Documents, including psychiatric screening assessments and clinic policy consent forms will be assigned to the patient portal for completion. All documents must be completed and submitted electronically a minimum of 24 hours prior to scheduled appointment time. If not submitted by the deadline, Paradigm Psychiatry may cancel the appointment and replace with a patient in need from our waiting list.
- New Patients should arrive 15 minutes early for check-in with a photo ID, insurance card and a form of payment. Payment is due at the time of service. Paradigm Psychiatry will collect an estimated amount based on patient's insurance benefits and eligibility coverage, that may be a deductible, copay, or coinsurance.
- The Self Pay rate for a 60 minute Intake is $250.00.
- New Patients arriving over 10 minutes late may be asked to reschedule. A $75 fee will be applied to compensate for the provider's scheduled time that cannot be replaced with another patient on short notice.
